Chapter Officer Information


All chapters that have annual receipts of $25,000 or less must file form 990-N (e-Postcard) with the Internal Revenue Service. This form must be filed on the 15th day of the 5th month after the chapter’s year end. In order to file this form you will need the following:

• Chapter’s Tax Id number (If you do not know this number please contact Headquarters and we will provide it to you)
• Chapter’s tax year
• Chapter’s legal name and address
• Any other name that the chapters does business under
• Name and address of a officer of the chapter
• Website address if the chapter has their own website
• Statement that gross receipts are $25,000 or less for the tax year
• If applicable a statement that the chapter has terminated
